Yes, you can, Create your own PDF or DOC templates and email them to support@hypesystems.com. We will test your templates and load them to your account if approved. We also provide template creation services for $85 for each PDF page.
Use the instructions below to create your own simple PDF or DOC templates.
Creating PDF templates with fillable fields linked to data in a database involves several steps. Below is a guide using Kofax Power PDF 5 Standard. Note that steps might slightly vary based on software versions.
Prerequisites:
Unlocked PDF Form:
- Ensure you have an unlocked or unprotected PDF form with fillable fields.
PDF Editing Software:
- Install and have access to PDF editing software. In this case, Kofax Power PDF 5 Standard.
Open the PDF Form:
- Launch Kofax Power PDF 5 Standard.
- Open the PDF form you want to edit.
Select Tool:
- Click on the select tool (black mouse pointer) in the left toolbar. This tool is used to select and manipulate form fields.
Access Field Properties:
- Double-click on the field you want to link to the database. This opens the Text Field Properties dialog.
Place the Tag:
- In the Text Field Properties dialog, locate the option for setting the field name or tag. This is where you link the field to the database.
- For example, for a field labeled "Patient’s First Name," you can place the tag
patientfirstname
in the appropriate field.
Repeat for Other Fields:
- Repeat steps 3-4 for each field you want to link to the database.
Save the PDF Form:
- Save the changes made to the PDF form.

Create a New Document:
- Open a new document in Microsoft Word.
Insert Tags:
- Insert placeholder tags where you'd like your HYPEMedical data to be inserted. Use a format that is easily identifiable and unlikely to be confused with regular text.

is a placeholder for the date of birth.
Save the Template:
Save the document with a name that indicates it's your generic template (.doc).
Saving as your generic .doc template ensures that the original document remains unchanged, and you can reuse it for multiple patients.
